Somerset treatment escalation plan

Advance care planning for people in the last years of their life will improve using a Somerset county-wide treatment escalation plan ‘STEP’ form. The STEP form can help us shape conversations and enhance patient care in all settings in Somerset.

Somerset Treatment Escalation Plan (STEP)

A new STEP form has been developed by NHS Somerset FT and is due to be released from June 2024.

 

e-STEP

We are developing a digital version of the STEP (we are calling it the e-STEP) to be hosted on our Somerset Shared Care Record, SIDeR+. This will be released in a phased approach starting with pilots in late summer 2024. Having a digital version of the STEP means that it can be viewed, edited or created in one place by anyone involved in your care such as doctors, nurses, carers or hospice staff. This means that you should not have to repeat the conversation with multiple services. Read more about SIDeR+ and how you can get involved here.
