If you have a long term condition, are otherwise unwell, or have wishes around hospital or other treatments it is even more important.

Having a conversation with your health and care team and your loved ones to work through your wishes using a treatment escalation plan will help to make these important decisions regarding what is right for you. It will also help community and hospital staff to understand and respect your values and preferences.
