Talking about end of life
Although sometimes difficult, it is important to think of the care and treatments we would, or would not like to receive if we became ill.
My Local Health Services
It is important, although sometimes difficult, to think of the care and treatments we would, or would not like to receive if we became ill.
If you have a long term condition, are otherwise unwell, or have wishes around hospital or other treatments it is even more important.
Having a conversation with your health and care team and your loved ones to work through your wishes using a treatment escalation plan will help to make these important decisions regarding what is right for you. It will also help community and hospital staff to understand and respect your values and preferences.
We have put together this patient information leaflet about treatment escalation plans. It may not answer all your questions about treatment escalation plans but it should help you to think about the issues.
For more information, speak to any members of the health and care teams looking after you. They will make sure you get the right support.
